Coaches Risk Management Handbook:

·         Each coach (football and cheer), will need to read the attached and complete the acknowledgement page (last page of the attached; electronically signed is acceptable)

·         Please collect each acknowledgement and have it included in the team book under the documents for coaches.

Volunteer Application Form: each volunteer needs to complete the attached Volunteer Application Form (Associations are to keep this for your records)

Background Checks:

·         Each volunteer needs to have a completed background check done (and cleared) before participating in activities

·         Association Affidavit: Once you have completed all background checks, the person that was responsible for conducting the checks should complete the attached Association Affidavit and return to my attention
